NDIS reports for plan reassessments.

What are they and why are they useful?

What is an NDIS plan reassessment?

An NDIS plan reassessment occurs before your NDIS plan end date to decide if there are any changes required for the next portion of support. 

The NDIS plan reassessment team should contact you via phone 3 months before your NDIS plan end date. They will have a conversation with you over the phone, provide details of the plan reassessment and work with you to set a meeting date.

The NDIS planning team will be wanting to obtain information and evidence about the services and supports that you have accessed in the current plan period. They will want to know what has worked, what hasn’t and any changes that you want to make in your next plan period, this is where the NDIS reports come handy.

The NDIS will want evidence that the supports and services utilised have been working towards and helping you to achieve your NDIS goals and that these supports are valid to include in your future NDIS funding. The way the NDIS gains this evidence is through reports from registered therapists and any other service providers that you have accessed. 

What information is included in NDIS reports your therapist?

 NDIS reports will include the following information about the person that is accessing the therapy such as:

  • Name;
  • Date of birth;
  • NDIS number;
  • Representative names;
  • Therapist name;
  • Professional qualification;
  • Professional peak body registration;
  • Trading business details.

The therapist will then provide information and data about:

  • Background information about the person accessing services;
  • A summary of the supports utilised;
  • Evidence in regards to the goals and any outcomes achieved;
  • Evidence based assessments that have been conducted and explain these results in regards to the support received;
  • Additional information;
  • Future recommendations;
  • Summary of support hours for the upcoming plan period.

A report from your therapist will provide evidence based information and data to show how the therapy has assisted you in working towards your NDIS goals. Within these NDIS reports your therapist and providers can make recommendations for your supports and services within the future.

Important information to note

  • Set up a meeting with your therapist to ensure that any requests you are seeking are included in the report;
  • If you have a therapy team you can request that they have a collaborative meeting to ensure that they cohesive in their approaches to get the best outcome for your next plan period;
  • Please give your therapist enough notice (3-6 weeks) to be able to produce a fantastic report that has evidence to justify future funding.
